The Comprehensive Health Assessment

You should write a summary of the health of the patient and develop a plan of care (2-3 pages, double spaced), which includes health risks, resources needed, and client education necessary to promote health. You may select the format of this summary and plan of care. Minimally, you should identify two measurable goals (at least one that is preventative/primary care-related) for the client, based on the data you collected during the comprehensive health assessment.

Use published evidence (guidelines from reputable/specialty organizations and/or research) to justify the plan of care you develop and the goals that you identify for the client. Identify interdisciplinary team members that should be engaged in the client’s healthcare, explain their roles as members of this patients’ healthcare team and give rationales for your choices.
